Bryan-
Please find attached the deed restriction as we discussed on the phone.The restriction must be printed, signed and
dated by you as the personal representative. The signed original then needs to be recorded at the Barnstable County
Registry of Deeds and a copy returned to our office prior to release of the Title 5 Certificate of Compliance. You had
advised me that you would coordinate with your realtor to process the deed restriction for you.
I have also spoken with B& B Excavation and advised them that the Title 5 installation permit is being released from our
office to allow the installation to move forward while the deed restriction paperwork catches up.
Please let me know if there are any additional questions and I will anticipate hearing from your realtor soon on the
restriction paperwork.

Good afternoon Amy,
I just left a message to you with Mary Ellis at the Health Department. I am the Personal Representative of the
Estate of leda Rodrigues, my mother, who owned a house located at 8 Howland Ct in West Yarmouth.
B&B Excavation was to perform a fix on the septic system to meet the selling guidelines, as the house was to be
sold this week. However, they never received a permit to do so. Christine in your office told me that the deed
restriction wasn't done correctly, and that your office mailed a letter to the West Yarmouth address, however,
the house is unoccupied and I live in Florida full time.
My question is - where do we go from here? What do I need to do to get the permit, or what do the realtors,
B&B, the engineer, or anyone else?
CC'd on this email are Darci and Dan, our realtors from the Cape, as well as Suzan, my sister. Please give me a
call of reply back to discuss.

Dave-
I have made some changes to the restriction to correct the Registry information. The book and page numbers you had
listed belonged to a property in Wellfleet. Also,you may want to check with B& B Excavation to find out if who they are
working with because we just had a letter returned to us regarding the failed septic report stating"undeliverable as
addressed"and it listed the owner as Estate of leda Rodrigues.
Once you have confirmed the owner or responsible person that can legally sign the deed restriction, please correct the
form, if needed,and return it t to me and I will have Bruce sign it and I will get it back to you for processing with the
owner.

Hi Dave-
This plan is all set for approval. The lot is located in the Zone II and will need a deed restriction. I have attached the new
form which is also posted on our website for future use. The form is a Word document with fields to be filled in with
the appropriate information. Once you have the form completed, Bruce will need to sign it and it will be returned to you
for processing with the owner. A copy of the recorded deed restriction will need to be returned to our office prior to
issuance of the installation permit. (We changed our policy on timeframe for releasing the permit due to complaints
from an installer whose COC was being held up because we hadn't received the recorded deed restriction yet.So,we
will not release a permit for the installation until all paperwork is processed ahead of time.)
Let me know if you are having any trouble with the form.There are instructions on the website that indicate you need to
click into the field and type a space to eliminate the text in the field if the field should be left blank.
