1. CONTRACTOR SHALL NOTIFY "DIG -SAFE" AS REQUIRED PRIOR TO THE START OF ANY
EXCAVATION WORK. CONTRACTOR IS RESPONSIBLE FOR PROTECTING UTILITIES WITHIN THE
WORK AREA DURING CONSTRUCTION.
2. THIS PLAN IS FOR SEWAGE DISPOSAL SYSTEM DESIGN PURPOSES ONLY. THIS PLAN IS NOT
FOR USE TO DETERMINE PROPERTY LINES. PROPERTY LINES PER SUBDIVISION PLAN OF
RECORD AND "SKETCH PLAN OF LAND IN YARMOUTH, MA. DATED 4/13/79 BY S.R. SWEETSER
ENGINEER ON FILE AT THE YARMOUTH TOWN HALL.
3. CONTRACTOR SHALL DETERMINE THE LOCATION OF EXISTING UNDERGROUND UTILITIES PRIOR TO
EXCAVATION
4. ALL CONSTRUCTION SHALL CONFORM TO THE STATE SANITARY REGULATIONS (310 CMR 15.00)
AND ALL OTHER APPLICABLE LOCAL, STATE, AND FEDERAL CODES AND REGULATIONS.
5. CONTRACTOR TO VERIFY EXISTING CONDITIONS, INCLUDING ELEVATIONS OF EXISTING BUILDING
SEWER, AND REPORT AND DISCREPANCIES TO THE DESIGN ENGINEER PRIOR TO THE SETTING
OF ANY SEWAGE DISPOSAL SYSTEM COMPONENTS.
SEWAGE DISPOSAL SYSTEM NOTES:
1. H-20 RATED COMPONENTS SHALL BE REQUIRED IN ANY AREAS SUBJECT TO VEHICULAR
LOADING AND AS REQUIRED ON THE PLAN.
2. THE DESIGNER WILL NOT BE RESPONSIBLE FOR "HE SYSTEM AS DESIGNED UNLESS
CONSTRUCTED AS SHOWN. ANY CHANGES OR DEVIATIONS FROM THIS PLAN MUST BE
APPROVED IN WRITING.
3. ALL GRAVITY SEWER PIPE SHALL BE 4" DIA. SCH 40 PVC UNLESS OTHERWISE NOTED. THE
MINIMUM SLOPE OF 4" DIA. SCH 40 PVC SHALL BE 0.01 FT/FT.
4. SOIL ABSORPTION SYSTEMS MUST BE VENTED WHEN DISTRIBUTION LINES EXCEED 50 FEET IN
LENGTH, WHEN LOCATED IN AREAS SUBJECT TO VEHICULAR LOADING, AND WHEN PRESSURE
DOSED.
5. FINISHED GRADE SHALL BE A MAXIMUM OF 36" AND A MINIMUM OF 9" OVER THE TOP OF ALL
SYSTEM COMPONENTS.
6. ALL ABANDONED SEPTIC SYSTEM COMPONENTS SHALL BE PUMPED DRY AND FILLED WITH
CLEAN SAND OR REMOVED AND REPLACED WITH CLEAN SAND UNLESS NOTED TO BE REMOVED.
7. ALL SEPTIC COMPONENTS SHALL BE INSTALLED WATERTIGHT.
8. COMPONENTS TO BE PROVIDE WITH WATERTIGHT ACCESS COVERS RAISED TO WITHIN 6" OF
GRADE WHERE REQUIRED.
9. PRIOR TO FINAL INSPECTION BY THE ENGINEER AND THE BOARD OF HEALTH ALL SYSTEM
COMPONENTS MUST BE INSTALLED INCLUDING ALL ACCESS COVERS WITH RISERS.
10. EXISTING SEPTIC TANK: HOMEOWNER SHALL REMOVE DECK FROM OVER THE EXISTING SEPTIC

CONSTRUCTION.
11. SOIL REMOVAL: ALL FILL, SUBSOIL (B LAYER), & SUBSTRATUM (Cl LAYER) SHALL BE
REMOVED FOR A DISTANCE OF 5' FROM THE SOIL ABSORPTION SYSTEM DOWN TO THE CLEAN
SAND LAYER, LAYER C2, APPROX. 89" BELOW GRADE. AREA TO BE BACKFILLED WITH CLEAN
"TITLE 5" SAND AND COMPACTED TO MINIMIZE SETTLING. CONTRACTOR TO N011FY TOWN
AND/OR ENGINEER TO VERIFY SOIL REMOVAL AS REQUIRED.
12. PERC ® INSTALLA11ON: AT THE 11ME OF SOIL REMOVAL VERIFICATION THE ENGINEER SHALL
CONDUCT A PERCOLATION TEST, WITNESSED BY THE YARMOUTH BOARD OF HEALTH, IN THE C2
LAYER. CONTRACTOR SHALL COORDINATE SCHEDULING OF YARMOUTH HEALTH AGENT AND
ENGINEER. PLEASE ALLOW 48 HOURS NOTICE.
13. CONTRACTOR SHALL VERIFY INVERT ELEVATIONS PRIOR TO INSTALLATION OF ANY SEPTIC
SYSTEM COMPONENTS.
