Hi Dennis-
Our office has reviewed and approved the floor plan and the revised septic plans submitted for the proposed
new construction at 2 Bass River Parkway. Prior to issuing the septic system installation permit, your septic
installer, Sean Farrell Excavation, will need to take and pass the Yarmouth Installer Exam and apply for the
installation permit (completed Title 5 Application, Trench Permit Application, and $ 110.00 permit fee, one
additional septic plan to be returned to the installer with the installation permit).
The extra site plans and floor plans, stamped with our approval, can be picked up at our office to include with
your Building Permit package to be submitted to the Building Department for review and approval. The
Transmittal Sheet to be included with the Building Permit package currently has Sean Farrell Excavation as the
applicant for the Building Permit. If this is not correct, the form can be altered with the correct
contractor/builder name or a new one can be completed when your paperwork is retrieved from our office for
submittal to the Building Office.

Hi Stephanie-
have reviewed the site plan for this property with the Building Permit application. Prior to final approval, the
following items will need to be addressed.
✓ Add a note: Not designed for a garbage disposal.
Add a note: Electrical permit required.
ee3. We will need specs on the portion of the retaining wall acting as a breakout barrier for the septic system
0 on the westerly side of the lot. The specifications can be added to the current septic plan or can be
submitted as a separate sheet (letter size is fine if easier). At a minimum, the following information is
needed: type of materials for wall (poured concrete, pin and block, etc.), liner information on inside of
wall, type of footing, elevations of top and bottom as currently shown on plan. Our in-house policy
typically allows timber/railroad tie walls for projects that are 2' or less.
_ LA. Inspection schedule for septic installation: stake retaining wall, inspect soil removal, inspect wall footing
for location and depth, final septic inspection upon completion.
