Dear Mr. Coleman,
We have been notified by Mrs. MacLelland, owner of 11 Collingwood Drive, that
your company installed an additional overflow leaching pit to the
septic system located on her property. The date of the installation was
. August 16, 1984.
A review of this department's files reveals that no as-built card was
• submitted showing the location of the septic system your company installed at
11 Collingwood Drive. The department also could not locate a copy of the
Disposal Works Construction Permit issued to you. The as-built must also show
the components of the existing septic system. Please send this office the
as-built card within one week of receipt of this letter.
If you should have any questions or comments relative to this matter, please
contact me at the Health Department, (508)398-2231, ext. 241, during the
office hours of 9:00-11:00 a.m., Monday through Friday.
