Dear Sir:
On July 29, 1987, this office conducted a final inspection of a septic
system installed at the above-referenced location. At the time of the
inspection, it was noted that the original septic system permit had been
reviewed on September 11 , 1986. In the Town of Yarmouth, disposal works
construction permits are only valid for six(6) months. This permit,
then, had expired on March 11 , 1987. This must be reapplied for.
Please be advised that as of July 30, 1987, the fee for disposal works
construction permits for new construction has been increased to $75.00.
In Addition, we have yet to receive an "as-built" card from the
installer.
Therefore, until the above is accomplished, this office will neither
issue a certificate of compliance nor forward any approval for issuance
of an occupancy permit.
