Box 771 West Barnstable, MA 02668 RE: 112 Main Street, Yarmouth Port, MA Dear Attorney Sundelin: The Health Department is in receipt of a subsurface sewage disposal inspection report for the above said address, completed by Down Cape Engineering dated November 11, 1996 and December 2, 1996. The report states that the cesspool is not in ground water and meets the minimum criteria for passing a septic. This letter supersedes the Health Departments letter of August 13, 1996. The report listed your property as needing further evaluation, after reviewing the report a correspondence letter was sent to Michael DeDecko on June 6, 1996 advising him to pump the cesspool to determine if ground water • was present and reevaluate with a revised report. Since the cesspool pumping and the revised report have not been performed, the report is considered failed by this department. You have sixty days to obtain engineered plans and upgrade your system to meet the State Title V requirements. Repair work that is done on your system must be performed by a licensed septic installer. If you should have any questions or comments relative to this matter, please contact me at the Health Department. Box 2384 Mashpee, MA 02649 Re: 112 Main Street (Route 6A) , Yarmouthport Subsurface Sewage Disposal System Inspection Dear Mr. DeDecko: This department is in receipt of a subsurface sewage disposal inspection report on the above noted property, conducted June 5, 1996 by your company, and received on June 24, 1996 by this department. The department has reviewed the report and determined that the cesspool must be pumped to determine if the cesspools location is in ground water, as there is 12 inches of affluent currently within the cesspool and the house is vacant. A review of the departments records reveals clay soil conditions with high ground water and several of the houses within this area have mounded and pumped septic systems. After the,(esspool has been pumped and reevaluated a revised report must be submitted to the Health Department. If you have any questions regarding this matter, please contact me at the Health Department.
