J.) LOCATION OF UTILITIES 15 APPROXIMATE AND CONTRACTOR SHALL BE RESPONSIBLE FOR
DETERMINING THE LOCATION OF ALL UNDERGROUND AND OVERHEAD UTILITIES PRIOR TO
COMMENCEMENT OF ANY WORK. THI5 INCLUDES, BUT 15 NOT LIMITED TO, REQUESTS TO DIG5AFE,
ANY PRIVATE UTILITY COMPANIES, AND THE LOCAL WATER DEPARTMENT. CONTRACTOR SHALL BE
RESPONSIBLE FOR VERIFYING AND PROTECTING ALL ABOVE AND BELOW GROUND UTILITIES PRIOR TO
AND DURING CONSTRUCTION.
2.) ALL COMPONENTS SHALL BE RATED FOR H-20 WHEEL LOADS IF UNDER PAVEMENT OR DEEPER
THAN 3'IN DEPTH.
3.) ALL WORKMANSHIP PROVIDED SHALL BE IN CONFORMANCE WITH THE SPECIFICATIONS OF THE
DPW OF THE TOWN OF YARMOUTH.
4.) fL0WA51 F FILL SHALL BE USED AROUND ALL CATCH BASIN GRATES AND A5 BACKFILL FOR ALL
ROAD CROSSINGS 50 AS TO MINIMIZE SETTLEMENT.
5.) THE LEACHING CATCH BASINS SHALL BE SET INTO CLEAN SAND. THE TOWN OR ITS
REPRESENTATIVE SHALL INSPECT THE SOILS AROUND AND BELOW THE LEACHING FACILITY PRIOR TO
FACILITY INSTALLATION.
G.) SOILS CAN BE VARIABLE AND TEST HOLE DATA 15 NO GUARANTEE OF SOIL CONDITIONS IN OTHER
AREAS. IF SOILS DIFFER FROM TH05E SHOWN IN THE SOILS LOGS, DESIGN ENGINEER IS TO INSPECT
THE SOILS PRIOR TO PROCEEDING WITH INSTALLATION OF ANY INJECTION WELL COMPONENTS.
7.) THE BOARD OF HEALTH REQUIRES INSPECTION OF ALL CONSTRUCTION BY AN AGENT OF THE
BOARD OF HEALTH AND THE DESIGNER. THE DESIGNER SHALL CERTIFY IN WRITING THAT THE INJECTION
WELL SYSTEM WAS INSTALLED IN ACCORDANCE WITH THE TERMS OF THE PERMIT AND THE APPROVED
PLANS. 48 HOURS ADVANCE NOTICE IS REQUESTED.
8.) INSTRUMENT SURVEY CONDUCTED FOR PROPOSED WORK ONLY. SITE PLAN SHALL NOT BE USED
FOR STAKING, OR ANY OTHER PURPOSES.
9.) THIS PLAN DOES NOT CERTIFY, GUARANTEE OR WARRANTY COMPLIANCE WITH DEEDED OR ZONING
BYLAWS, SPECIFICALLY, BUT NOT LIMITED TO, SIDELINE SETBACKS AND BUILDING HEIGHT
RESTRICTIONS. OWNER 15 RESPONSIBLE FOR OBTAINING SUCH A DETERMINATION FROM THE
APPROPRIATE AUTHORITY,
