IDear Board of Health Department,
Welcome to our "Challenger Sports Massachusetts Board of Health Application and
Supporting Documents" packet for 2019. We have updated our policies/procedures to
reflect the recent changes in the state regulations,. We have also made updates to the
layout of our packet based on feedback we received. We have made a conscious effort
to help streamline the inspection process by writing our policies/procedures alongside
the inspection form.
Within this packet you will find our "Site Specific Policies and Procedures", copy of our
2019 Insurance Certificate, Health Care Consultant Agreement, Written Orders from our
Health Care Consultant as well as other supporting documents. The fee for the
application is also included. However, should it be missing then please let us know.
Please note, this packet does not contain the following documents/information:
• Staff's CORI
• Staff's SORI
• Staff's physical and Immunization Records
• Camper records (The collection of camper records is an ongoing process)
• Camp Director/Camp Health Supervisor (Allocated 2 weeks prior to camp)
All missing information listed above can be presented in person. Please reach out to
schedule date to review and we will make ourselves available.
