Dear Mr. Arista: •
MassDEP had an engineer from this office conduct an inspection of the wastewater
treatment facility("WWTF")for the above referenced location on May 7,2019.
The purpose of the inspections was to witness the clear water test and to determine if the
WWTF was constructed as approved by the Department. It was concluded, at the time of the clear
water test,that the WWTF was constructed in compliance with the Department's approved plan with
some deficiencies. The Department has since received certification from your engineer that those
deficiencies have been corrected. Therefore,the Department grants you permission to operate the
wastewater treatment facility. Please be advised that all the conditions of Groundwater Discharge
Permit 969-OM1 are in effect.
