Dear Mr. Kasper,
Thank you for your efforts regarding the cleanup at the above address. After our phone
discussion on Friday, Dec. 7 1 made a follow-up visit to the property to view the work
you stated was completed on Dec. 6. While there was an improvement over the
condition I observed on Dec. 5 work remains to be done in order to bring the property
into compliance with public health regulations.
Findings
The following were noted on the property during my site visit of Dec. 7:
1) Trash and discarded items stored outside (including, but not limited to paper,
plastic, electronic components, cardboard, broken glass, discarded roof shingles
and small household items)
2) Tire stored outside — left side of property
3) Tarps on ground — left side of the property
4) Trash barrel uncovered and filled to the top with wa r — right side of property
5) A discarded mattress in the left rear of the property with odent holes in it
6) A discarded propane tank in the right rear of the property
Please find photos of the property attached.
Violation
Due to the abovementioned conditions the property is in violation of:
1) Massachusetts State Sanitary Code, 105 CMR 410

Corrective Measures
Please correct the aforementioned conditions within 10 days from the receipt of this
letter in order to avoid possible fines and/or court action. In the absence of a timely
correction of the violations listed the Town may be forced to have the corrective
measures completed with a lien placed on the property for the incurred cost. Upon
completion of corrective measures please contact the Health Office for a review of the
status of the property.

The Yarmouth Health Dept. has viewed the above property in response to a complaint
received on Oct. 5.
Findings
The following were noted on the property during a site visit:
1) Trash and discarded items stored outside
2) Tires stored outside
3) Loose tarps stored in such a way that they can collect rainwater
4) Receptacles that are either accumulating or can accumulate rain water
5) Carpet outside collecting rain water
6) Fallen rotting apples
7) Gasoline containers and other containers of toxic or hazardous materials

Violation
Due to the abovementioned conditions the property is in violation of:
1) Massachusetts State Sanitary Code, 105 CMR 410
2) Town of Yarmouth, Board of Health, Handling and Storage of Toxic or Hazardous
Materials Regulation

Dear Ms. Philips,
In response to your concern of children residing in a house with loose trash outside, yard debris, rats and
unregistered vehicles I visited the above property with Health Director Bruce Murphy on 8-30-2018.
Findings
The following were noted on the property during a site visit:
1) Toys and bicycles outside in the yard
2) Overgrown vegetation throughout the property
3) 3 unregistered vehicles
4) Kiddie pool with some water— possible mosquito breeding ground
Several knocks on the door went unanswered. No loose trash was observed on the portion of the
property we were able to view. We could not determine whether or not rat activity was present due to the
overgrown vegetation. The property. has been the source of numerous complaints to the Health Dept.
regarding trash and rodents over a number of years.
Based on the presence of children's toys and bicycles we recommend your department conduct a
welfare check as it appears children reside on the premises.
Please let our dept. know your findings after you make your visit to the house. Thank you for your kind
consideration of this matter.

The Health Dept. visited your property on the afternoon of Thurs., Aug. 30 in response
to complaints of trash, rats, debris and unregistered vehicles on your property. Several
knocks on your door went unanswered.
Findings
From the street and on the way to your front door observations were made. The
following were noted on the property:
1) Overgrown vegetation throughout the property
2) 3 unregistered vehicles
