The renewal license applications for Funeral Directors have been enclosed for your completion.
Please fill out the two applications and return them with the license fee to the Yarmouth Health
Department. The fee is $55.00 per license application ($110.00 total), and the check should be
made payable to the Town of Yarmouth.
