Dear Commission Members and Administrator Kelly Grant:
This letter requests the closing out the above Order of Conditions on this property.
This consisted of a septic upgrade, and to the best of my knowledge was completed
according to the conditions. An as built site plan dated August 18, 1987, was also
completed. We hereby request the issuance of the Certificate of Compliance at this
time.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
