Dear Attorney Alva:
This is to serve as a follow up to our June 13,2019 inspection of 9 Pinewood Road,West Yarmouth,attended by Mr.Juan
Marichal,Wiring Inspector Ken Elliott,Plumbing/Gas Inspector Lee Hall and myself.
The following represents a list compiled by all inspectors based on observations on that day:
SECOND FLOOR UNIT
-Remove electric stove and associated outlet.
-Remove kitchen sink. Requires plumbing permit.
-Repair exterior door—loose hinges
-Remove lock from interior door at stairs or remove door.
-Expose concealed wiring in walls and ceiling.
-No exterior light at exterior stairs.
-Expose piping in bathroom.
-Enclose open risers on exterior stair system.
BASEMENT
-Expose concealed wiring in walls and ceiling.
-Remove dead bolt from basement door.
-Dryer vent penetrates a wall and is not solid metal. Replace vent.
-Open splice box.
-No handrail on exterior basement stairs.
-Variable rise on exterior basement stairs(top riser).
-Pump room—extensive black mold. Pump is not operating correctly—water on floor in basement areas.
-Exposed wire splice in furnace.
-Sump pump wire is not fastened.
-Remove paint can away from furnace.
-Obtain Gas/Plumbing Permit for Water Heater.
-Provide additional combustion air for water heater.
-Furnace fan runs continuously.
-Remove kitchen sink and cabinets.
-Remove framed walls.
-Septic alarm equipment light was showing a trouble condition. Have equipment checked for proper operation.
Our records show that no Building,Plumbing or Wiring Permits were applied for,or issued for the alterations to the second
floor and basement. Therefore permits are now required with associated inspections.
NOTE:Because additional bedrooms were created on the second floor,without benefit of a Building Permit,and will now
become lawful, via the submission of a Building Permit application and the issuance of same,all Smoke Alarms shall be
hard-wired and interconnected. All required Carbon Monoxide Detectors shall be placed in the required locations,including
the basement.
Finally,this house is to remain a single family dwelling.
