RE: Maintenance and Service Contract for the Waterloo Biofilter System (Nitrogen Reducing)
located at 300 Long Pond Drive, Yarmouth
Dear Barbara:
Clear Water Industries (CWI)proposes to provide the service and maintenance for the Waterloo
Biofilter System located at the above referenced property. The following maintenance and service
schedule is for the next two (2) years of operation commencing upon receipt of signed contract&
Annual Cost paid in full.
Scheduled Semi-Annual Service:
Cost: 2 inspections @ $250 per visit = $ 500.00
2 field effluent tests (included above)
2 Total Nitrogen samples @ $105 each = $ 210.00
Barnstable County Innovative/Alternative Database management fee = $ 50.00
Total Annual Cost =$ 760.00
(Note: Access cover for all components must be at the ground surface.)
1. Check sludge and scum depth in the septic tank and clean the biotube filter as needed.
2. Check panels and alarm system.
3. Check ejector pumps and float switches in both the Pump Chamber and the Waterloo Biofilter.
4. Check spray nozzles, fan and foam cubes in Waterloo Biofilter.
5. Flush laterals in leach field as required.
6. Take effluent sample as required by Massachusetts D.E.P. Sample will be analyzed for the
following:
*Dissolved Oxygen,
*Turbidity,
*Total Nitrogen (TN = TKN +NO3 +NO2),
and *pH.
7. Notify Client verbally of any problems encountered.
(Note: There may be instances when the high water alarm will sound. In the event of an alarm
condition, you are requested to silence the audible alarm and contact
CWI @ (978) 356-0779 for instruction and/or a follow-up field visit by a CWI personnel.

I have read and agree with the above Scope of Work, including the granting of access to the subject
property in order to conduct the required maintenance:

Unscheduled service:
1. Unscheduled emergency service calls will be billed at the following hourly rates:
*Monday through Friday 7am—5pm: $90
*Monday through Friday 5pm—7am: $150
*Saturday and Sunday: $150
with a minimum of a 2 hour charge.
2. If results of field effluent testing for pH, Dissolved Oxygen or Turbidity do not comply
with Massachusetts Department of Environmental Protection limits, additional testing for
Total Suspended Solids and Biochemical Oxygen Demand would need to be done at a
certified laboratory at a cost of$105 per sample. If a high test result is obtained for Total
Nitrogen, additional testing would be necessary at a cost of$105 per sample. If all the
above tests need to be performed at a certified laboratory, the cost would be $220. Owner
would be contacted prior to any resampling.
3. In the event of an alarm or system failure CWI may be required to take immediate
corrective measures (i.e. pumping septic tank). The cost of these corrective activities will
be billed accordingly.
Certified technician:
The service technician shall be a Massachusetts Certified Operator. The certified operator will
be David F. Clark, George F. Norris, Mark S. Cottrell, or Mario Rosa.
Reporting requirements:
In accordance with DEP's Title V Regulations, CWI will file semi-annual reports transmitting
the data from the semi-annual sampling of the effluent, as noted above, as well as a review of
any unscheduled service. CWI will also file semi-annual reports with the home owner and the
local Board of Health.
