On 1/29/15, the Commission issued an Order of Conditions for the
above -referenced site, for the construction of a 2 car garage.
Photographs and an as -built site plan are enclosed for review.
Instead of gutters and downspouts, the garage roof run-off is
handled and contained within the shell base which surrounds the
structure and is part of the completed shell driveway.
To the best of my knowledge and belief, this is to certify that
the site is in substantial compliance with the approved plans
and Order of Conditions. We hereby request the issuance of a
Certificate of Compliance at this time.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
