You just need to come by for a building permit sign off. You can fill in a transmittal sheet and I will sign it. The forms are
at my window.

Good Afternoon,
Please submit the Building Application along with sign offs from the Health, Conservation & Water Departments, two
Certified site plans of the property showing the addition with the setbacks to the property lines, 2 sets of construction
plans stamped by a Structural Engineering, Rescheck IECC 2015 and the recorded decision from Board of Appeals. There
is a $35.00 application fee.

Please see Linda Cipro, Rosa Fallon or Tim Sears when you have the required sign offs from other departments and the
appropriate application fee.
