(BSC) is pleased to submit this Request for Certificate of Compliance (COC) Application on behalf of Paul Butler (the Applicant) for the property located at 25 Parkers Neck Road, South Yarmouth, Massachusetts (the Site). The proposed work involved the construction of a single-family dwelling with a garage, Title V septic system, fill, utilities, and associated driveway. On September 5th, 2019, BSC Group conducted a site inspection to confirm that all permitted work reflects the permitted work included under the Order of Conditions (OOC) and the Minor Modification submitted on July 11, 2019 (plan of the same date), and that Resource Areas have not been negatively impacted as a result of the permitted work. All areas are currently stabilized and the work pertaining to the OOC, DEP File #SE 83-1965, issued on 2/23/2016 and extended on 11/10/2018, were conducted in significant compliance. For more information, please refer to the attached As-Built Plan and WPA Form 8a.
