VIOLATION:
Failure to obtain a Building Permit for the installation of a Spa/Pool in the basement of the house
located at 161 Center Street, Yarmouth Port MA.
Re: 780CMR, Massachusetts State Building Code:
Section R105.1 Permits,Required
Section R114.1—Violations, Unlawful Acts
The first Violation Notice was dated June 12, 2019. However, as of today's date you have failed
to respond. In addition, I have been advised by the property owner that she spoke with you
concerning this matter and you advised her that she was responsible for filing for the Building
Permit. As a Construction Supervisor and installer,you are the responsible party.
Accordingly, you are hereby ordered to submit the required permit application, immediately. You
may obtain the appropriate forms on line,via the Town of Yarmouth,Building Department website.
Failure to do so by July 8, 2019 will result in appropriate enforcement action.
