O'HEARN, SURVEYOR Tc-lephone (508) 394-1265 Fox (508) 398-4057 DEC 14 1995 HEALTH DEPT. This office inspected the sanitary system at the above referenced site on November 20, 1995 and again one week later. On November 20th I went to the site and found that the excavation for both the primary and the reserve areas had been done. I found the area shown on the plan for the primary system to have clay withal 12" of the bottom of the excavation. The area of the reserve was much better, and I dug down by hand 30 " in good medium to coarse sand. I stayed there while the contractor tried to excavate the primary area. As he was removing the clay, he encountered a very large rock which he was unable to move. After excavating around the rock, it was decided to install the system in the area of the reserve in order to avoid blasting and possibly damaging the foundation. All excavated material was replaced with clean medium sand from a source off site. During my next inspection, the system was installed and I verified the amount of 1/8" to 1/2" washed stone and visibly checked the quality of the washed stone. They both appeared to be satisfactory. I then checked the pipe for scope and found it met the required minimum pitch. Very truly yours, R.J. O'HEARN, SURVEYOR Ric and J. 'Heam, President
