HomeMy WebLinkAboutIncident Report INCIDENT REPORT FILE DATE: August 4, 2019 C 0 PY TIME: 0815 Hours NAME: Janet Gilmore ADDRESS: 'P; On this date&at this time I was requested, by Yarmouth Fire Department, to respond to a structure fire at 80 Mayflower Terrace. When I arrived I reported to Lt. Scott Smith and he showed me into the house. I looked over the bedroom where the fire was believed to have started. I observed a water bed,with a heater, an air conditioner, which had fallen from the window, and a toaster oven.The receptacle, on the exterior wall, had an adapter to increase the number of available female receptacles used to plug in additional appliances.There were remains of three male plugs plugged into the adapter.The adapter was severely damaged. The remains of several extension cords were found in the same bedroom. All of the cords were very small gauge wire of the light duty lamp cord type. I could not see the cord for the toaster oven and could not tell if it was plugged in. Some of the extension cords were run along the floor and/or stapled up and around the window casing. At least one of the extension cords was jammed under a drawer in a built in cabinet. I opened the receptacle, on the wall,to check for any evidence of the fire starting in the wall or due to faulty wiring.The wires attached to the receptacle were wrapped in the incorrect direction around the screws of the receptacle but there was no visible overheating or arcing within the box or on the receptacle itself. I could not gain access to the basement to check the wiring further.The exterior service and metering was destroyed. I could not determine the cause or origin of the fire but I believe it to be an overheated electrical system. Respectfully submitted, G K.
