Re: William Coldrick, Septic System Installation
Lot 5,Smiths Point Road, Great Island, West Yarmouth,MA
Dear Bruce:
On Thursday, October 26, 1995,this office performed an inspection of the installed
sewage system septic tanks and related equipment at the referenced site in order to certify
to a satisfactory installation.
At that time,we made a recommendation that the sanitary tees in the septic tank on the
west side be trimmed to the lengths as called for on the septic plan. We understand that
this was done. Also at that time, a recommendation was made to provide a more practical
cover for access to the septic tank on the east side. We do not know if this was done,
however,we presume it was. None-the-less,we wish to certify, based upon the inspection
made on October 26th combined with our inspection results of 1993, that the sewage
system has been installed in conformance with the plans as approved.
Please don't hesitate to call me directly should you have any questions. Thank you for
your cooperation with the project.
Very truly yours,

Re : Coldrick Leaching System Excavation
63 Smith Point Road , Great Island , West Yarmouth , MA
Dear Mr . Mason :
On December 9 , 1993 , we inspected the above referenced excavation .
The installer had done an exceptionally good job of removing the
unsuitable material for 10 feet around the proposed leaching
system to a depth of approximately 9 ' at the southeast side and
approximately 12 ' on the northwest side . In the excavation , the
contractor temporarily opened a hole down to elevation = 2 , 4 feet
below the proposed bottom of the leaching system. We encountered
clean , medium fine sand to satisfy Note 17 on the proposed sewage
disposal system plan concerning soil quality verification below
the leach pit .
Very truly yours ,
