Item Description Cost Total
Title 5 All labor&materials for the installation of a Title 5 septic system,as per plan dated
5.17.18 by Ronald Cadillac.All workmanship&materials shall conform to D.E.P.and
Town of Yarmouth rules and regulations.
Also Included Septic and Trench permits. Pump and remove or backfill existing septic system. Remove
all necessary trees. Remove unsuitable soil as per plan and replace with clean sand. Install
Work Limit and set up site visit with Conservation Agent. Sleeve sewerline over waterline_
A licensed electrician to wire all components to existing panel. Construct the wall as per
plan with liner. Tank to be H-20 two compartment with liner. Will locate existing utilities
to try and work around. Will coordinate relocation if needed. Proposal does not include
utilities if they need to be changed. All disturbed areas to be backfilled and graded.
Loam and Hydro-seed If all disturbed areas are to be loam&hydro seed,an additional charge of $800.00 will be
added to this proposal.Owner is responsible for maintaining&watering.Chase&
Merchant is not responsible for run off or inclement weather.(PLEASE INITIAL IF YOU
ELECT THIS ADDITIONAL CHARGE )
Trench permit If you are choosing Chase&Merchant for your installer,please sign&date Trench
application enclosed.
Payment Terms Half the total amount due upon acceptance of proposal.Balance due at completion of work_
Finance charges of 18%per annum will be applied to past due amounts on a monthly basis
at 1.5%,plus all collection costs,including attorney's fees. IF ESCROW TERMS APPLY:
Signed proposal MUST INCLUDE payment terms and contact information of attorney
holding funds in escrow.
Chase&Merchant is not responsible for any underground utilities,water/irrigation lines or tanks not marked by Dig-Safe or
homeowner.We are not responsible for TREES,BUSHES,SHRUBS OR PLANTS,WALKWAYS OR PATIOS unless specified in
writing by homeowner. We will notify Dig-Safe. All material is guaranteed to be as specified.All work to be completed in a
workmanlike manner according to standard practices. Loam&seed will be an extra charge,not included unless specified. We are
not responsible for driveway damage due to equipment/machinery.We are not responsible for a lic.plumber or electrician unless
specified.Our workers are covered by Workman's Comp.Ins.We Propose hereby to furnish material and labor,complete in
accordance with above specifications,for the sum below. Acceptance:The above prices,specifications and conditions are
satisfactory and are hereby accepted.You are authorized to do the work as specified.Payment will be made as outlined above.
