On December 12, 2017, the Commission issued an Order of Conditions, since Amended, for the above -referenced site, for the construction of an addition, rock retaining wall and driveway alterations. A site visit was been made for the purpose of closing out the Order of Conditions. An as -built of the site was performed; the plan is enclosed as part of this filing. The rock wall at the edge of driveway was constructed further from the wetland and the footprint smaller than what was proposed. The driveway, consisting of gravel, is slightly modified from what was proposed. This is to certify to the best of my knowledge and belief, with the exception of the above -listed minor deviations, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
