Unless you have a variance request to your abutter's foundation,the variances you mentioned can be approved in-
house by staff as long as the variances are listed on the plan. There is no additional application required. If there is a
variance to the property line,you will either need to show the abutting foundation with the setback to the new leach
facility,or add a note "no abutting foundations within 20'of the proposed leach facility".

The system as designed is going to require a few variances(property line & basement setback and<5'clearance to GWT
in<2 mpi soils); should we complete the variance application form and E-mail it with the plans?

We use a standard Title 5 application that is 1 page with a carbon copy. The septic installer usually completes the
application and submits it with the plans and payment directly to the Health Departmentfor-review-and=approval. I-am
not expecting you to complete the form as the engineer. Please feel free to submit a copy of the plan to the Health
Department to get it into our review pile ahead of the installer's submission if you choose. A copy of the plan can be
emailed if that is easiest.

Good afternoon Amy—
We're working on the septic upgrade design for 47 Broadway, and are looking to prepare the application.We've got a
few examples of application forms for other properties that are different than the attached MADEP form. I've looked on
the Town's web site for the local application, but have not been able to find it.
Could you direct me to the correct local form on the web site,or provide one to me via E-Mail?
