Thank you, the engineer will add that statement, to the letter and fix the dep number and recording
reference. I will send in the new letter and hard copies of both with the check. I will most likely overnight it
to you on Monday so you have in plenty of time. Is the $75 to cover the recording costs do you record the
original compliance?

Hi Shannon
The letter needs to be corrected to reference the correct Order and should make the statement certifying
substantial compliance with the approved plans and the Order of Conditions.
We also need to receive a check for $75 per the checklist.

The next meeting is actually December 5 and I am happy to add this request to that agenda if I receive the
complete request early next week. You are not required to attend the meeting though it is recommended if
there are any questions raised during my review. You can check in closer to the meeting to see if there are
any potential issues.

Thank you again for all your help in tracking down and sending the information and Compliances for the
above property you have been a huge help.
We respectfully request to be put on the agenda for the next Conservation Commission meeting which I
believe is December 19, 2019. Please confirm that someone does not need to be present at the meeting to
obtain the Certificate of Compliance for the Order of Conditions for DEP File No SE83-1896 on the above
property.
I have attached the Request for the Certificate of Compliance Form 8A and also as you requested a letter
from an engineer to be submitted at the meeting.
Please confirm this is acceptable and you do not need anything further and that this is on the agenda for
the next meeting.
