Dear Ms. Grant:
Below please find the following for filing on behalf of the Bass River Yacht Club, Inc. at
22 Frothingham Way, South Yarmouth, MA 02664:
1. Request for Certificate of Compliance for SE83-62 (incl. an engineer's statement)
with 7 copies; and
2. Request for Determination of Applicability in connection with the rebuilding of the
foundation beneath the clubhouse kitchen (incl. a detailed narrative and plan) with 7
copies, proof of certified mailing to the Mass. DEP, and waiver of 21 day period.
Also, enclosed please find a copy of the Certificate of Compliance for SE83-1085 which
was issued on October 16, 1992, but unfortunately not recorded at that time. After a diligent
search of the Club's records, the original has not been located. Can the Commission or Town
Clerk issue a new or certified copy which I could then proceed to record?
And lastly, in connection with the Notice of Violation issued to the Club in the Fall, the
sod along the back side of the bulkhead has been removed and the American Seasgrass is soon to
be planted.
As Vice Commodore of the club, I want to thank you in advance on behalf of the Board
of Governors for your assistance, and for marking the enclosed for hearing on the next available
hearing date.

beneath the Club kitchen &food preparation area. The net building`�.i

work area is approx. 25S of the entire building -
By the very nature of the proposed work the exterior perimeter of the
foundation x111 require excavation sufficient to shore the building
and install a new continuous concrete footing and reinforced masonry
foundation wall. Due to the high water table this foundation will be
by necessity a shallow hearing -foundation. In order to deal with water
and frost the footing will be founded on a continuous crushed stone
pack placed and compacted to elevate all of the concrete work equal to
or above the current water table levels.
Equipment and construction materials will use the existing concrete hard-
stand for storage and as a work platform. This area resides within the
work limit 6 erosion control line.
The material excavated will he reused as the backfill once the foundation
work is complete. Any excess excavation soils will be removed from U19
the site. New trucked in material will consist of the crushed stone peck
used for the new concrete footing sub-base.
Current and continued storm water r=o-off consists of attenuated surface
water management and remains on-site.
CONSTRUCTION PROTOCOL/14—FTHODOLOGY
q Work Limit fox construction is effectively the 3 -soaped panels betwee:-i the

fencing for delineation and erosion control
s Strip, and stockpile top-soil loams and lawn squares as practical. The top-
soil is intended for re-use. The vegetated grass will he spoiled off-site
yard in order to clean away old mortar- Stockpile for re-use
s San•-cut and jack hammer the section of concrete apron that runs adjacent to
the Southeast facing kitchen foundation wall. Excavate base and sub-base
to establish ;.iter level and verify coarse sandy bearing. strata
3 Norking along the rest of the kitchen perimeter mini-excavate the 41m3' box
trench to allow for cribbing, support beams and foundation repairs. It
is noted that there will also be interior demolition to expose existing
floor joists and girt beam in the immediate kitchen area only.
e Remove the failed 'mock foundation along the 3 walls of the kitchen shoring
and cribbing the relatively light roof loads for the construction phase
:x Set grade control and install the compacted stone and formed concrete footing
while continuing with the interior framing repairs and upgrades. This
will include the new interior bearing girt reinforced concrete footing
tiorking along the footing line build the reinforced concrete block foundation
wails and re-attach the wood floor and wall systems. Add the course of
block for the attached shed and reconnect the wood framing there also
a Grade, compact and place the new apron concrete back to the building face
Replace the stogkplled fill and topsoil. Re-shape the grade and install tha
new ground cover landscaping
.� Complete any interior repairs and breakdown and cleanup the staging area
