RE: Subsurface Sewage Disposal System Inspection Report
46B Beach Road, West Yarmouth, MA
Dear Ms. Dawson:
This department is in receipt of a subsurface sewage disposal system inspection report dated
September 13, 2019 regarding the above referenced property, conducted by Joseph M Martins of
Accu Sepcheck. The inspector reported the property as having conditionally passed the
inspection, however,the following repair(s)must be made

V1. D-Box is fractured and deteriorating and needs to be replaced.
/2. Pump alarm audible is not working and needs to be fixed.
3. Septic Tank needs pumping as solids exceed 40% of tank volume.
4. Recommend pumping and rinse of chamber too.
Repairs must be performed by a licensed septic installer and a septic repair permit issued by the
Health Department. When the septic repair is made and the repair inspected by this department,
a certificate of compliance will be issued. The inspection report will then be able to be used for
purposes of real estate transfer. You have sixty (60) days to make the necessary corrections to
the septic system.

Department at the telephone number printed above on Monday through Friday from 8:30 AM to
4:30 PM.

in 310 CMR 15.303 or in 310 CMR 15.304 exist.Any failure riteria not evaluated are
indicated below.

replaced or repaired. The system, upon completion of the replacement or repair, as approved by
the Board of Health,will pass.

The septic tank is metal and over 20 years old*or the septic tank(whether metal or not) is structurally
unsound, exhibits substantial infiltration or exfiltration or tank failure is imminent. System will pass
inspection if the existing tank is replaced with a complying septic tank as approved by the Board of
Health.
*A metal septic tank will pass inspection if it is structurally sound, not leaking and if a Certificate of
Compliance indicating that the tank is less than 20 years old is available.
