VIOLATION NOTICE

Violation: Occupying a single family house without a Certificate of Occupancy
Re: 780CMR,Chapter 1,Section R111.1,as amended—Use and Occupancy,which provides that
"No building or structure shall be used or occupied —until the Building Commissioner has
issued a Certificate of Occupancy."
Re: 780CMR, Section 114.1, as amended—Unlawful Acts, which provides that "It shall be
unlawful for any person, or corporation to—occupy a building or structure in violation of the
provisions of 780CMR."
This department's records show that no Certificate of Occupancy has been issued for the house
located at 22 Mayflower Terrence permitted under Building Permit No. BLD-17-002394, dated
November 3, 2016, which is currently occupied. Our records reflect the last inspection was
performed on March 9, 2018, for insulation.
Accordingly,you are hereby ordered to initiate the process of obtaining this Certificate by contacting
this office and arranging for a final inspection. In addition, because there appears to be deviations
from the proposed front building elevation, shown on the plan dated August 16,2016, submitted to
the Old Kings Highway Historic District Committee and what has been constructed,you are required
to file a new Application for Certificate of Appropriateness and arrange for a hearing before the
Committee. All questions concerning this protocol are to be directed to the OKHHDC Department.
This action shall be taken immediately. Failure to do so will result in appropriate enforcement
action, which would include the issuance of Building Code Tickets, carrying fines of up to $1,000
per day.
