Hi Art
As this project is jurisdictional under the wetland regulations you will need a surveyed plan that shows the wetland
delineation and the setbacks to those wetlands. Is such a plan already with the Building Department?
Once you have the plan you will need to file a permit application with the Conservation Commission before proceeding.

Good Morning, Kelly please find transmittal information on hand written form along with partial site plan and print.
The building department has full documents hopefully you can acquire whatever information you need from them, I’ve
been working with Tim Sears and Linda Cipro On this permit application.
I apologize for not being able to print the form I don’t have printer access.
