In the course of reviewing open/pending Certificates of Occupancy, I find that the Certificate for the
new house constructed at 11 Thornton Brook Road under Building Permit No.BLD-16-002319 has not
been issued. Based on the inspection records a final inspection was performed on April 29, 2016 and
the required Air Leakage Teat results lettatwas not'provided. This would indicate a test was never
performed or the test result letter was never submitted. In either event, the house is being occupied
without a valid Certificate of Occupancy.
Accordingly,as the contractor or record,you are hereby ordered to take appropriate action by arranging
for this test and submitting the written results to this department no latter than March 30, 2018.
Finally, it is imperative you act on this matter, because the fact the house is occupied without a
Certificate of Occupancy, could impact the Home Owner's Insurance Coverage.

Re: 11 Thornton Brook Road West Yarmouth Certificate of Occupancy
Dear Mr. Hatch:
This is to serve as a follow up to my March 8, 2018 letter concerning the newly constructed single
family house at 11 Thornton Brook Road, West Yarmouth. In that letter you were advised to provide
the Air Leakage Test results for this house, but to date you have failed to do so. As a result, this
department has not issued the required Certificate of Occupancy.
Accordingly,you are hereby ordered to arrange to have this test completed and provide the test results to
this department, immediately. Failure to do so may result in the issuance of a complaint to the State
Construction Supervisor's Licensing Authority.
As stated in my previous letter, the lack of a Certificate of Occupancy could have an impact on the
homeowner's insurance.
