Hi Kelly! John and I have revised the MA DEP Transmittal form and have three checks ready to go out. Attached is the
revised form – we added Category 5a at 145 feet for a total of $580. Town and State each owed $290 for this (the 12.50
additions and subtractions taken care of with original submission). We made another check out for $290 for the local
CES for 145 feet x $2.00. The State check will go out this morning certified mail. Do you need us to hand deliver the
Yarmouth checks today? If so, to the front drop slot or the side drop box? Just let me know – thank you for your
patience with this.!!

I am just going through the application and noticed that there is no fee included to permit the CES. There is a $4/linear
foot and $2/linear foot fee under the state and local regs. Can you arrange for payment so I can determine the
application complete?
