Project Description 1. a. Area Description (use additional paper, if necessary): The subject upland property contains and existing residential house, septic system, and driveway connecting to Angus Avenue in South Yarmouth. This developed upland parcel of land is located within a 100 year flood zone, Zone AE Elevation 11, Land Subject to Coastal Storm Flowage, according to FEMA Map Panel 25001C0589J. Angus Avenue contains existing residential houses located on both sides of the street. c. Work Description (use additional paper and/or provide plan(s) of work, if necessary): Proposed 3 bedroom residential house, new Title 5 septic system, deck, landscape wall and landscape features,driveway, located at 14 Angus Avenue (Map 33, Parcel 46), South Yarmouth, MA. The new Title 5 septic system will replace an old, failed septic system located within this property. All proposed site alterations are located within areas of land that have been altered within the past. This developed property is classified as Land Subject to Coastal Storm Flowage,100 year flood zone, AE El. 11. There are no biological wetlands located within this subject upland parcel of land, and there are no biological wetlands located within 100' of the subject property. Tempoary erosion controls will be utilized during the construction phase of this proposed construction project. I further certify that the property owner, if different from the applicant, and the appropriate DEP Regional Office were sent a complete copy of this Request (including all appropriate documentation) simultaneously with the submittal of this Request to the Conservation Commission. Signature of Applicant Date Signature of Representative (if any) Date wpaform1.doc • rev. 7/27/2020 WPA Form 1 – Request for Determination of Applicability • Page 4 of 4 PROJECT DESCRIPTION – 14 Angus Avenue, Map 33, Parcel 46, South Yarmouth, MA The proposed project by the applicant, Marie Caron, includes the construction of a new 3 bedroom house, Title 5 septic system, deck, landscape wall, landscape features, and a driveway connecting to Angus Avenue. The project includes the removal of the existing house structure located within this residential property located at 14 Angus Avenue. All proposed construction activities within this subject property will be located within upland areas of land that have been altered in the past, by the construction of the existing residential structures located within this subject property. There are no biological wetlands located within this subject residential property, or within 100’ of this subject property. The subject property is located within a designated flood zone, 100’ year flood zone, AE Elevation 11, classified as land subject to coastal storm flowage. This section of Angus Avenue contains existing residential houses and buildings located on both sides of Angus Avenue, all located within the area designated as land subject to coastal storm flowage, AE Elevation 11. No biological wetlands and no wetland buffer zones will be altered by this proposed residential project. Proposed site alterations will occur within land subject to coastal storm flowage, since the entire developed property is classified as land subject to coastal storm flowage, 100 year flood zone, AE Elevation 11. The proposed residential project includes a new Title 5 septic system for improved treatment of wastewater, which will be an improvement for the local groundwater system, within this section of South Yarmouth. The proposed septic system will be reviewed by the Yarmouth Board of Health. Adequate erosion controls will be utilized to prevent any sedimentation from migrating off property, to adjacent residential properties, and into the adjacent roadway (Angus Avenue) from this proposed construction project. Construction Protocol 1. Remove existing structures from the property, remove to suitable upland location off property. 2. Install new Title 5 septic system, and leaching field. 3. Construct new 3 bedroom house. 4. Construct deck. 5. Construct landscape wall and landscape features. 6. Construct new driveway connection to Angus Avenue. Again, all proposed site alterations are located within upland areas of land that have been altered in the past, by past residential development located within this site at 14 Angus Avenue, South Yarmouth, MA.
