November 1,2018 Mr.Richard Garvey 25 Tee Way Yarmouth,MA 02664 Subject: Garvey Residence,25 Tee Way,Yarmouth,Massachusetts Dear Mr.Garvey: This office has completed our structural investigation and field inspection of the newly installed exterior OSB shear wall sheathing elements completed along the referenced residence in Yarmouth,MA on October 29,2018. Based the condition of the existing exterior OSB shear wall sheathing some areas along these elements need to conform to the Massachusetts State Building code with respect to structural adequacy. After new 8 penny nails are added as needed between (spaced equal distant between two 8 penny nails)the ineffective recessed 8 penny nails(countersunk below the exterior surface)as found in the field and the OSB shear wall sheathing areas will then conform to the Massachusetts State Building code with respect to structural adequacy. This evaluation was based on observed measurements along existing exposed (non- concealed)areas of the building found when a field reconnaissance and tactile inspection when completed by the engineer. If additional engineering data, plans and tests are brought to the engineer's attention in the future the recommendations and results presented herein may be altered as determined by the engineer. If you have any additional questions regarding this matter, please feel free to contact me at your convenience. Sincerely _tot ong4s, 4 O in 8 STRUCTURAL No.40f 7 Joseph D.Bianchi,P.E.,S.E.
