SLAB ASSESSMENT COULD BE CONDUCTED BY A QUALIFIED ENVIRONMENTAL PROFESSIONAL. SUBSEQUENTLY, ON 1/31/2020, FOLLOWING
ASSESSMENT OF THE SUBSLAB SOILS AT THE SITE, JOHN TADEMAWIELANNDT, LSP FROM BENNETT ENVIRONMENTAL SUBMITTED AN EMAIL
CONFIRMATION TO ANDREW COONEY MASSDEP/ER THAT A 120 DAY CONDITION WAS IDENTIFIED AT THE SITE WITH SEVERAL REPORTABLE
CONCENTRATIONS OF OHM IN THE SOIL THAT WERE CONFIRMED BY LABORATORY ANALYSIS. AS A RESULT A BWSC101 WAS ENTERED FOR THE
120 DAY CONDITION.
ON FRIDAY MARCH 13, 2020 THE LSP, JOHN T.W. CONTACTED DAN CRAFTON MASSDEP/ER SECTION CHIEF TO REQUEST THAT THE PRP BE ALLOWED
TO HAVE THE CLEANUP CONDUCTED AS AN LRA AS THE VOLUME OF SOIL IMPACTED IS EXPECTED TO BE LIMITED TO LESS THAN 5 CUBIC YARDS OF
SOIL. IN RESPONSE TO THE REQUEST, MASSDEP REQUESTED SUBSLAB ASSESSMENT DOCUMENTATION TO SUPPORT THE LRA REQUEST.
ON MAY 15, 2020 MASSDEP RECIEVED AN LSP RESIGNATION LETTER FROM LSP, JOHN D. TADEMAWIELANDT #1707, IN ACCORDANCE WITH 310 CMR
40.0169(2), AS NOTIFICATION THAT HE WAS NO LONGER THE LSPOFRECORD AND THAT HE HAD NO FURTHER ASSOCIATION WITH THE ABOVE
REFERENCED SITE.
ON JUNE 5, 2020 MASSDEP RECEIVED A MEMORANDUM LETTER FROM LSP, BILL HOYERMAN OF CIVIL & ENVIRONMENTAL CONSULTANTS, INC.,

CONTACTED DAN CRAFTON MASSDEP/ER SECTION CHIEF TO REQUEST THAT THE PRP BE ALLOWEDTO HAVE THE CLEANUP CONDUCTED AS AN LRA AS THE VOLUME OF SOIL IMPACTED IS EXPECTED TO BE LIMITED TO LESS THAN 5 CUBIC YARDS OFSOIL. IN RESPONSE TO THE REQUEST, MASSDEP REQUESTED SUBSLAB ASSESSMENT DOCUMENTATION TO SUPPORT THE LRA REQUEST.ON MAY 15, 2020 MASSDEP RECIEVED AN LSP RESIGNATION LETTER FROM LSP, JOHN D.
