A condition of the Order is that it be closed out with a Certificate of Compliance when the work is
complete. Here is a link to the filing forms (scroll down to Request for Certificate of Compliance)

As part of the request you will need a statement from a certified professional that the work was carried out per
the Order. Once I receive the request it will go on the agenda for review by the Conservation Commission.

Hi Kelly
My name is Kathy O'Malley. My husband and I are the home owners of 71 South St. Can you
please explain to me what a "Certificate of Compliance" entails? This message from Mark
Coleman was the first time I ever heard of this. How do I obtain the completed document? I'm
assuming that it has to do with my garage that we finished in 2016. I thought everything was
completed when I got the final sign off on my permit sheet. What am I supposed to do to close
this out?
I want to do whatever is necessary to obtain closure so we can move forward on the permit for
our inground pool.

Hi Mark
I have a building permit sign off for a swimming pool at the above address. You
will need to file an application with the Conservation Commission for this
project. Do you have a surveyed plan? Depending on the extent of the work a
Request for Determination of Applicability may be sufficient.
This property owner also has an open Order of Conditions for an earlier project
which needs to be closed out with a Certificate of Compliance (SE83-2012).
