All filings must be made on Town of Yarmouth forms. All applications must be submitted two weeks
prior to the next scheduled Conservation Commission meeting. No new information may be accepted
into the Commissioner's packet a week before the meeting. Failure to follow the Administrative
Checklist guidelines shall result in an Administrative Incomplete Application and will not be advertised
for a Public Hearing. Refer to the meeting and fee schedule. Contact the office if you need assistance.

7 copies of the first and second page of the application (double sided) with a comprehensive
and detailed narrative of the project including existing & proposed conditions, construction
sequence, type of equipment, staging locations, drainage and stormwater, erosion controls,
invasive species management and alternative analysis. The narrative shall document how the
project meets performance standards per 310 CMR 10.0 & TOY Wetland Bylaw, Chapter
143, BVW delineation sheets and other supporting information.
8 copies of the plan/sketch, folded separately, right side out with title visible and attached to
each application. All plans shall reference NAVD1988 unless otherwise noted. Landscape
plans shall be detailed to show proposed & existing conditions, native species, size and
spacing.
A pdf copy of the application, supporting information and plan/sketch emailed to
kgrant@yarmouth.ma.us.
1 copy of a Yarmouth Assessors locator map highlighting parcel(s) where work is proposed,
including map and parcel numbers.

PROJECT NARRATIVE
#51 Park Avenue, West Yarmouth, MA
This site consists of a developed 7,680 s.f. lot improved with a four-bedroom dwelling. It
is within an area of seasonal and year-round homes. The site lies entirely within Land
Subject to Coastal Storm Flooding (LSCSF). No other wetland resource areas or their
buffers apply.
The project involves renovating and expanding the existing dwelling on a new flood
compliant foundation. The existing dwelling is two stories with an attached deck, the
proposed dwelling will remain two stories and a portion of the deck is to be converted to
a new kitchen. The existing 4 bedroom septic system will remain and recently passed
inspection on 2/7/2020.
The plans are to be filed with the Zoning Board of Appeals due to the non -conforming lot
and setbacks.
Drywelis are proposed to capture all roof run-off. No import of fill is expected to be
required.
Land Subject to Coastal Storm Flowage
The site lies within Floodzone AE el. 12, the existing grades are
approximately el. 9 around the existing dwelling.
No fill is proposed, additional stairs will be required to access the raised first
floor height.
No adverse impacts are expected during any flooding event due to the large
scale of coastal events, and modest site changes.
