All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule
Failure to follow the Administrative Checklist guidelines shall result in an Administrative
Incomplete Application and will not be advertised for a Public Hearing.

On 8/24/2016, the Commission issued an Order of Conditions for
proposed work at the above -referenced site.
A site visit was made; an as -built plan and photos are enclosed
for review. The mitigation plantings have been completed and the
area calculated. Andrew Garulay, the Landscape Architect involved
with the mitigation plantings and plan, has supplied his
certification for this portion of the project.
To the best of my knowledge and belief, we hereby certify that the
work has been completed in substantial compliance with the
j'approved plan and Order of Conditions. We hereby request the
issuance of a Certificate of Compliance at this time.

The site was inspected by Andrew Garulay, RLA on December 2, 2020 for compliance regarding
the Buffer Restoration Plan dated 8/1/2016 and the Landscape Plan dated 10/6/2016.
Buffer Restoration Plan dated 8/1/2016:
The American Beach Grass is doing well and conforms to the plan. The layer of Sweet Fern is
doing very well and is becoming a contiguous stand. There were some Pasture/Carolina Roses
and Creeping Juniper that needed to be replaced along the way. Some of the Creeping Juniper
that were struggling were replaced with Blue Pacific Juniper (unintentional mistake by
contractor) a few years ago, but more recent replacements are native Creeping Juniper species
and this stand is on its way toward infilling fairly well. The Pasture/Carolina Roses will take a bit
longer to infill to a contiguous stand, but are heading in that direction. The Potentilla, Viburnum,
Inkberry, and Eastern Red Cedars are doing very well. The mitigation areas exceed what was
shown on the approved plan.
Landscape Plan dated 10/6/2016:
The Landscape Plan was done to add a small patio next to the house (Approved 10/19/20 —
Administratively?) and shows a 10' mitigation area. That area is planted with a successful stand
of Pasture/Carolina Roses consistent with the approved plan.
I believe that the project complies with the Order of Conditions.
