The septic system covered under this Order was completed shortly after approval in 1985. During the early 1.990's Ship Shops underwent a complete refurbishment. The septic tank shown on the 1985 plan remained in service and was connected to a replacement SAS that was located greater than 100 ft. from the river boundary, possibly within the pre -2014 flood zone. Only the septic tank approved under SE 83-575 remains in service today. Based upon my knowledge, information and belief, I certify that the septic system was installed in accordance with the Yarmouth Health Dept. standards and although modified since installation, in accordance with the Order of Conditions. The project warrants a Certificate of Compliance at this time. Please contact me directly should you have any questions. Sincerely, Cape Cod Engi Bing, Inc. Robert M.
