To the Conservation Commission,
Attached please find Form 8A for the Request for Certificate of Compliance for SE 83-
575 and a check in the amount of $75.
The septic system covered under this Order was completed shortly after approval in 1985.
During the early 1.990's Ship Shops underwent a complete refurbishment. The septic
tank shown on the 1985 plan remained in service and was connected to a replacement
SAS that was located greater than 100 ft. from the river boundary, possibly within the
pre -2014 flood zone. Only the septic tank approved under SE 83-575 remains in service
today.
Based upon my knowledge, information and belief, I certify that the septic system was
installed in accordance with the Yarmouth Health Dept. standards and although modified
since installation, in accordance with the Order of Conditions. The project warrants a
Certificate of Compliance at this time.

an approval of any plans stamped by a registered professional engineer, architect, landscape
architect, or land surveyor?

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
Office (see http://www.mass.gov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office-
for-your-city-or-town. html).
