This program of structural tests and inspections is submitted as a condition for issuance of the
building permit in accordance with 780 CMR 1703.0 of the 9th edition of the Massachusetts
State Building Code.
The following firms, agencies, or individuals (hereinafter referred to collectively as agents) will
perform the tests and inspections under the direction of the SER:

The following items of construction, if checked, are specified in the structural plans or
specifications on a performance basis. Their structural design will be reviewed by the SER and
their construction is included in the program for tests and inspections on the attached sheets:

Light Gage Metal Trusses
The following items are excluded from this program of structural tests and inspections, since
they are designed by other structural engineers not under the aegis of the SER and the SER
was not retained to provide performance specifications for their design. These other structural
engineers must be assigned by the owner, architect, or construction contractor, as applicable, to
be special SER’s for their respective designs and to provide a program of structural tests and
inspections for their respective designs.

In-Situ Bearing Strata for Footings
Item Agent Scope
1. Bearing Strata QC Review. GEO Review Contractors field quality control procedures.
2. General Excavation. TL/GEO (GEO) Inspect strata for conformance to the structural
drawings, specifications, and geotechnical report. (TL)
Ensure that excavation is to proper depth or material.
Ensure that excavation is controlled and contains no
unsuitable materials.
3. Bearing surfaces of footings. GEO Inspect bearing surfaces for conformance to the
requirements of the structural drawings, specifications,
and geotechnical report.
4. Rammed Aggregate
Foundations. GEO Determine that materials and procedures are in
accordance with the submitted plans, specifications
and geotechnical report.
5. Other. NR
Controlled Structural Fill (Prepared Fill)
Item Agent Scope
1. Controlled Structural Fill QC
Review. TL Review Contractors field quality control procedures.
2. Fill Material. TL Test material for conformance to specifications or
geotechnical report. Perform laboratory compaction
tests in accordance with the specifications to determine
optimum water content and maximum dry density.
3. Installation of Controlled
Structural Fill. (780 CMR

TL Provide full-time inspection of the installation, in
accordance with the specifications and 780 CMR Table
1704.7.
4. Density of Fill. TL Perform field density tests of the in-place fill in

Cast-in-Place Concrete Construction

Review frequency and scope of field testing and
inspections.
2. Mix Design. SER/TL Review mix designs prior to placement. (SER)
Verify use of original mix design. (TL)
3. Materials Certification. TL Review material certifications for conformance to
specifications.
4. Batching Plant. TL Review Plant quality control procedures and batching
and mixing methods.
5. Reinforcement Installation. TL Inspect size, spacing, cover, positioning and grade of
reinforcing steel. Verify that reinforcing bars are free
of form oil or other deleterious materials. Inspect
rebar laps and mechanical splices. Verify that bars
are adequately tied and supported on chairs or
bolsters.
6. Anchor Rods. TL Inspect anchor rods prior to and during placement of
concrete.
7. Post-Tensioning Operations. NR
8. Formwork Geometry. TL Inspect form sizes for proper sizes of concrete
members, types, alignment and cleanliness.
9. Concrete Placement and
Sampling Fresh Concrete. TL Observe concrete placement operations. Verify
conformance to specifications including cold-weather
and hot-weather placement procedures. Perform
slump, density and air content tests at point of
discharge. One set of cylinders per 50 cubic yards or
portion thereof. One set of cylinders per 5,000 SF of
floor or wall area or portion thereof.
10. Evaluation of Concrete
Strength. TL Test concrete compressive strength (ASTM C31 &
C39), slump (ASTM C143), air-content (ASTM C231
or C173) and temperature (ASTM C1064).
11. Curing and Protection. TL Verify conformance of curing, cold weather and hot
weather protection procedures for all concrete.
12. Welding Reinforcing Steel. NR
13. Mechanical Reinforcing
Splices. TL Verify proper embedment, joint fit-up, and tightness of
mechanical parts.

specifications. Verify cleanout hole locations (high lift
grouting). Verify the installation of bond beams and
special shapes.

Inspect reinforcing steel for size, quantity, condition
and placement for conformance to responsible RDP
approved submittals and Contract Documents.
Inspect welding of reinforcement and review welder's
certifications. Inspect mechanical splices.
7. Grouting Operations.
TL
Continuous Inspection of grouting procedures for
conformance with the specifications. Inspect cells
prior to grouting. Assure observation holes have been
installed prior to high lift grouting.
8. Weather Protection.
TL Inspect protection for cold and hot weather for
conformance with the specifications.
9. Anchorage.
TL
Inspect anchorage of concrete masonry to other
construction for conformance to the Contract
Documents. Inspect anchorage of veneer masonry to
metal framing and other construction for conformance
to the Contract Documents.

Steel Construction

6. Bolting. TL Verify bolt size and grade. Verify installation and
tightening of high-strength bolts per AISC
requirements for snug tight bearing bolt applications.
Verify that splines for TC Bolts have been separated
from bolts.
7. Welding. TL Visually inspect all welds. Verify size and length of
fillet welds as follows;
(Field Fillet Welds - 100% visual, 15% magnetic
particle.
Shop Fillet Welds - 25% visual, 10% magnetic
particle.)
Full-penetration welds. Inspect pre-heat, post-heat
and surface preparation between passes. Frequency
as follows;
(100% continuous visual inspection and 100%
ultrasonic for all shop and field welds.)
8. Shear Connectors. TL Inspect size, number, positioning and welding of shear
connectors. Inspect suds for full 360 degree flash.
Ring test all shear connectors with a 3 lb hammer.
Bend test all questionable studs to 15 degrees
9. Structural framing, Details and
Assemblies.
TL Inspect for size, grade of steel, camber, installation
and connection details. Check against approved
construction documents and shop drawings.
10. Open Web Steel Joists. NR
11. Metal Decking. TL Inspect attachment and side-lap fastening of metal
roof and floor deck.
12. Field Correction of Fabricated
Items.
TL Review documents of responsible RDP approved
repair and verify completion of repairs.

Cold Form Metal Framing
Item Agent Scope
1. Member Sizes TL Randomly verify exterior wall framing members for
size, spacing and general fit-up. Exterior wall framing
shall be checked for conformance with contract
documents, manufacturers shop drawings and
requirements. (Provide weekly observations while
erection of exterior wall framing is in progress.)
2. Material Thickness TL Randomly verify the gage of wall framing members
with contract documents, manufacturers shop
drawings and specification requirements.
(Provide weekly observations while truss erection is in
progress.)
3. Material Properties SER / TL Review submissions for material properties of cold-
formed steel materials for bearing wall framing. (SER)
Randomly verify the material grade is in conformance
with approved shop drawings.(TL)
4. Mechanical Connections TL Verify exterior wall framing attachments and
connections to main structure.
5. Welding TL Field welding of cold formed steel shall not be
permitted without the written approval of the cold
formed steel Design Engineer and the SER
6. Framing Details TL Review for conformance with the contract document
and approved submitted shop fabrication drawings.
(Provide weekly observations while erection of exterior
wall framing is in progress.)
7.Trusses TL Verify that the permanent individual truss member
restraint/bracing are installed in accordance with the
approved truss submittal package.

Sprayed Fire-Resistant Materials
Item Agent Scope
1. Material Specifications ARCH Review for material certifications for conformance to
specifications.
2. Laboratory Tested Fire
Resistance Design
TL Review UL fire resistive design for each rated beam,
column, or assembly.
3. Schedule of Thickness TL Review approved thickness schedule.
4. Surface Preparation TL Inspect surface preparation of steel prior to application
of fireproofing
5. Application TL Inspect application of fireproofing.
6. Curing and Ambient
Condition
TL Verify ambient air temperature and ventilation is
suitable for application and curing of fireproofing.
7. Thickness TL Test thickness of fireproofing (ASTM E605). Perform a
set of thickness measurements for every 1,000 SF of
floor and roof assemblies and on not less than 25% of
rated beams and columns.
8. Density TL Test the density of fireproofing material (ASTM E605).
9. Bond Strength TL Test the cohesive/adhesive bond strength of
fireproofing ASTM E736). Perform not less than one
test for each 10,000 SF.
