B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional

On 6/17/77, the Commission issued an Order of Conditions for
the above -referenced site, for the repair/modification of an
existing pier system and obtain local approval for submittal for
licensure with the Ch. 91 program.
A site visit was made and an as -built performed for the purpose
of closing out the Order of Conditions. Photographs and the as -
built site plan are enclosed for review.
Most of the pilings appear to be original. The fixed pier is
substantially within the area that was originally approved. A
boardwalk, ramp and pilings were added over time, shown in
photos and on the site and septic plan which was approved in
2009 and for which a Certificate of Compliance issued on
2/4/2011. Since then, a float and another piling were added.
During the house construction, the (then) building inspector
required the builder to repair the pier since the decking was a
hazard. He replaced the decking with Azek.
To the best of my knowledge and belief, this is to certify that
the ..pier system is in substantial compliance with the Order of
Conditions and plan with the exception of the following.
® The replacement decking is 0.9' wider and does not have V"
spacing (though all site work received Certificate of
Compliance in 2011, photo enclosed)
® There is an added float piling and float (not deployed at
time of survey)
• There are a couple of 1.8 sf cantilevered platforms at the
end of the fixed portion, to support utilities.
Believing these deviations to be minor in scope, we hereby
request the issuance of a Certificate of Compliance at this
time.
