HomeMy WebLinkAboutDYIMS_PE-CDW_Site_report_20210610 Architect: Perkins Eastman DPC Distribution: Commodore PMA Consultants Sandra Cashen File ARCHITECT’S SITE OBSERVATION REPORT DATE OF VISIT: 06/10/2021 PROJECT: Dennis Yarmouth Intermediate Middle School 286 Station Avenue South Yarmouth, MA 02664 DATE OF ISSUE: 06/22/2021 PROJECT NO: 71011 WEATHER CONDITIONS: Sunny, mid 70’s ARCHITECT: Perkins Eastman, DPC 20 Ashburton Street 8th floor Boston, MA The purpose of the Site Observation Report is to record general observations of the work in progress at the time of the Architect’s visit. WORK IN PROGRESS: 1) Installation of construction trailers 2) Earth moving for soil materials 3) Grading for the site, athletic fields, building structures including foundations and slabs on grade. 4) Preparation for excavation for the Rammed aggregate piers (RAP contractor due on site late in the week of June 13). FIELD OBSERVATIONS: PE visited the site to review ongoing progress with the site work. 1) Site clearing including stump removal appears to be almost complete. Smaller organic debris/materials are all that remains to be removed. 2) Based on visual observations, all of the indicated construction fences and erosion control structures have been installed. Grading is ongoing. No issues were observed. 3) Ryan DiRico, the Commodore Superintendent, showed PE and CDW an abandoned monitoring well that was discovered within the limit of work (in the building footprint). PE concurs with CDW that this should be demolished and removed according to local, state and federal regulations (see photo below). 4) The structures for the abandoned septic system that were found onsite (within the limit of work) have been removed by the site contractor RBO. 5) Ryan DiRico showed PE and CDW the remaining structure for the abandoned septic system that was found outside the limit of work. Per CDW it is acceptable to cut and cap the feeds and leave this structures in place (see photo below and also attached CDW field report). 6) Commodore pointed out the exact location of the conduit containing the high speed fiber connection between the admin building and the high school (see photo below and RFI #050). 7) Ryan DiRico also showed PE and CDW some existing features along Station Avenue that appear to be in conflict with the new work and that will either necessitate some adjustments in the layout of the work, or will need to be relocated: a. Telephone pole impinging slightly upon new curb cut at new east entrance from Station Avenue. b. Existing manhole cover that will straddle the edge of the new sidewalk (see photos) c. Two telephone poles at the west end of the site frontage that impinge upon the 8’ wide sidewalk as currently located (see photos below) In most cases these conflicts can be resolved through slight layout adjustments. Commodore will submit this as an RFI. Weather Sunny – 75 degrees Observations Based on visual observations, erosion control measurements have been installed on the perimeter of the entire site. No issues were observed. The existing well appears to be an abandoned monitoring well. The recommendation is to demolish this well according to the local, state, and federal regulations. The abandoned septic system that was found onsite has been removed inside the limit of work per the contractor.
