The purpose of the Site Observation Report is to record general observations of the work in progress at the time of the Architect’s visit.
WORK IN PROGRESS:
1) Excavation for underground infiltration structures
2) Installation of UGI-7
3) Marguerite commencing work
4) Trench excavation and installation of water main at west entrance drive up to site entrance
FIELD OBSERVATIONS:
PE visited the site to review ongoing progress with the site work.
1) At the time of observation, RBO was excavating and preparing the footing line from A/D21 to A/D25 (area D), including
excavating, compacting subgrade, and installing crushed stone base. [According to the attached GPI field report for 7-06, by
end of day RBO had prepared the footing line up to coordinate C/d24.7.]
2) Marguerite is mobilizing on site and was working on making forms for footings
3) Excavation was commencing for UGI-1 at the main parking area (see photo).
4) Observed installation of the DMH-2 inlets at the underground infiltration structure UGI-7 (at east access drive--see attached
photo).
5) Rustic Fire Protection with RBO were in the process of excavating and installing pipe to extend the water main up to about
350’ from station avenue, up to the site entrance gate (see photo).
6) The Southern part of parking lot grade has been prepared up to about UGI-1 (see photo below)
7) The grade for the access loop road appears to be prepared more or less up to the SW corner of UGI-3.
8) The installation of VSCs by CNC was still suspended at the time of the observation (travel day for CNC crew).
9) According to Ryan DiRico of Commodore, the limit of work perimeter and tree line will need to be moved outward by about 6’,
at least at critical areas (UGI-2, westernmost access loop road, etc.)

This Report is a summary installation list of installed vibratory stone columns, (VSC), in accordance with Submittal #31
3400-02.0 31 3400 – Aggregate Piers, prepared by CMC Professional Services LLC, 335 E. Geneva Rd #3116, Carol
Stream, Il 60188.
Glossary of Header Terms:
VSC Pier #: Each pier is identified by number according to location plan included in the above referenced
Submittal.
Proposed Depth: Total VSC depth from working grade to proposed bottom elevation as put forth in design
criteria.
Installed Depth: The as-built length of pier as constructed from working grade to termination in suitable native,
(bearing) soil.
Stone Take: The amount of Crushed Stone used in constructing a given Pier, (expressed in volume), one cubic
yard weighs approximately 1.2 Ton.
Comments: Note any deviation from standard installation procedures, proposed depths or changes in location of
said Pier.

FOOTING PREPARATION:
THE SEQUENCE OF PROCEDURES ARE IDENTICAL FOR ALL INSTANCES OF PREPARATION, AND ARE AS FOLLOWS:
Excavate to six inches below proposed Bottom of Footing Elevation
Compact the exposed subgrade, Piers now revealed – with a heavy vibratory plate compactor
Place six inches of 3/4” Crushed Stone upon the Prepared Subgrade
Compact said Crushed Stone
Crushed Stone Base now ready for Footing installation
Activity for Thursday, July 6th, 2021:
Footing Subgrade Compaction / Preparation Detail :
The soil matrix encountered at subgrade elevation largely consists of uniform sands and fine sands with scant traces of a
non‐plastic silt. Therefore, the proper in‐situ compaction of this material is largely dependent on the relative moisture
content of the soil. Since the encountered materials are of native, (natural) deposition the water content can be variable
from location to location. Each situation must be evaluated to determine that the proper soil compaction techniques are
utilized vis‐à‐vis the moisture content of the soil. In other words, it may be necessary to moisten some areas with
supplied water in order to certify proper compaction in these circumstances.

Dennis Yarmouth Intermediate Middle School
Yarmouth, MA
On Wednesday, June 30, 2021, our office visited the job site to observe the
construction progress for the Dennis Yarmouth Intermediate Middle School.
The Construction trades working this day included Site contractors. During the
visit, I meet with Paul Genereux, Site Superintendent and maple syrup
enthusiast from Commodore Builders.
The site is cleared of trees, and the topsoil is stockpiled to the North-East corner
of the site. Rammed Aggregate pier installation is in progress with one
installation machine. Excavations for footings have just started at the North-West
Corner of area D. Stone is placed over the excavated surface. A representative
from GPI was monitoring the activity.
I attended a foundation pre-construction meeting, along with representatives
from the concrete installer, concrete supplier, general contractor, owners
project manager, and Andrew from Perkins Eastman. The meeting covered
mobilization, testing, schedule, and coordination of the foundation installation.
In general, the work observed appears in conformance with standard industry
practice. There were no apparent deficiencies noted.
