The purpose of the Site Observation Report is to record general observations of the work in progress at the time of the Architect’s visit.
WORK IN PROGRESS:
1) Excavation and subgrade prepping for foundations in Area B
2) Placing concrete for walls and footings (D and C)
3) Setting forms and rebar for footings, and piers at Area C
4) Vibratory stone column installation
5) Backfilling of of sewer force main connection
6) Ongoing work on VSC installation
7) Ongoing layout work by survey crew.
8) Road grading and general site prep
FIELD OBSERVATIONS:
PE visited the site to review ongoing progress with the site work and foundations.
1. RBO has prepared Footing line B22 and B25 from K to area C and B and by the end of the day on 7-28 they
prepared lines J/B30 to J/B32 and J.7/B35 to J.7/B33, as well as north south return at line B33 (see also attached
GPI report).
2. Most of the walls and piers at Area D have been stripped and are ready for backfill and Marguerite is making final
concrete placement preparations at the foundation wall from D.3 to D.24 for a concrete pour today.
3. Concrete is also being placed today for footings on line E (area C)
4. At area C Marguerite is in the process of tying rebar for footing, pier and wall placements. Footing forms are being
prepared on line H and line C21 (south elevation and around south east corner) for placement later this week, and
many interior column footings in area C have are formed and tied.
5. The pump station for sewer force main is in place; force main has been installed from lift station to limit of work
(on 7-27) and RBO is in the process of backfilling.
6. CNC is installing vibratory stone columns at southwest part of Area A(see photo); VSC installation is due to be
completed today.
7. Brennan is continuing to install controls for anchor bolts at piers in Area D
8. Processed gravel has been backfilled above cultic chambers at UGI 5.
9. Commodore is planning on having some additional trees removed as necessitated by the correction of the limit of
work fence along the west edge of the site.
10. PE observed the stockpile of recycled glass for backfill around utility pipes; the new delivery appears to be mostly
glass and glass detritus (see photo); there is little odor from the pile.

Dennis Yarmouth Intermediate Middle School

On Wednesday, July 22, 2021, our office visited the job site to observe the
construction progress for the Dennis Yarmouth Intermediate Middle School.
The Construction trades working this day included Site contractors. During the
visit, I spoke with representatives from Commodore Builders and PMA. Andy
Hazelton and I walked the site and attended the weekly owner meeting.
Aggregate pier installation is in progress with one installation machine.
Excavations for footings are continuing into Area C and will be following the RAP
installation South. Stone is being placed over the excavated surface, and a
representative from GPI was monitoring the activity.
Concrete installation is in progress. Area D footings and walls are in various stages
of completion. Interior footings and piers, the North and East footings and walls,
and the West footings are installed. Formwork is in place along the West wall,
and reinforcing is installed. The observed reinforcing size, spacing, and cover are
in conformance. I had a brief conversation with the reinforcing foreman to
review his general procedures and found them to be acceptable.
The concrete visual quality of the finished concrete is good, with no voids or
indications of formwork failures. Anchor rods for the building columns are set
with plywood templates, and the exposed threads are protected with heavy tape.
The formwork for the column footings was simplified at line A columns, d6 and
d8. Additional plain concrete was placed between the column footings creating
a monolithic placement. The contractor was informed that any deviation should
be brought to our attention prior to procceding with the work. We have reviewed
this condition, and it is acceptable as installed.
Additional concrete is placed at the footing elevation under the exterior aprons.
This additional concrete will simplify the wall formwork and is acceptable.
Footing penetrations were reviewed with Paul Genereux. Final underground
coordination will require a wall footing penetration at line c21 between columns
F.5 and G. The wall footing penetration detail can be used at this location. It will
be acceptable to eliminate the wall footing between the faces of the column
footings.

The schedule was reviewed with the commodore. There is a need to backfill the interior area without
backfilling the exterior, creating an unbalanced load on the foundation wall. We asked that the contractor
submit an RFI with this question. We will review and provide direction through the RFI response.
In general, the work observed appears in conformance with standard industry practice. Other than the
items noted above, there were no apparent deficiencies noted.

Activity for Friday July 23rd, 2021: AREA C
Footing Subgrade Compaction / Preparation Detail :
The soil matrix encountered at subgrade elevation largely consists of uniform sands and fine sands with scant traces of a
non‐plastic silt. Therefore, the proper in‐situ compaction of this material is largely dependent on the relative moisture
content of the soil. Since the encountered materials are of native, (natural) deposition the water content can be variable
from location to location. Each situation must be evaluated to determine that the proper soil compaction techniques are
utilized vis‐à‐vis the moisture content of the soil. In other words, it may be necessary to moisten some areas with
supplied water in order to certify proper compaction in these circumstances.

The soil matrix encountered at subgrade elevation largely consists of uniform sands and fine sands with scant traces of a
non‐plastic silt. Therefore, the proper in‐situ compaction of this material is largely dependent on the relative moisture
content of the soil. Since the encountered materials are of native, (natural) deposition the water content can be variable
from location to location. Each situation must be evaluated to determine that the proper soil compaction techniques are
utilized vis‐à‐vis the moisture content of the soil. In other words, it may be necessary to moisten some areas with
supplied water in order to certify proper compaction in these circumstances.

Glossary of Header Terms:
VSC Pier #: Each pier is identified by number according to location plan included in the above referenced
Submittal.
Proposed Depth: Total VSC depth from working grade to proposed bottom elevation as put forth in design
criteria.
Installed Depth: The as-built length of pier as constructed from working grade to termination in suitable native,
(bearing) soil.
Stone Take: The amount of Crushed Stone used in constructing a given Pier, (expressed in volume), one cubic
yard weighs approximately 1.2 Ton.
Comments: Note any deviation from standard installation procedures, proposed depths or changes in location of
said Pier.

Excavate to six inches below proposed Bottom of Footing Elevation
Compact the exposed subgrade, Piers now revealed – with a heavy vibratory plate compactor
Place six inches of 3/4” Crushed Stone upon the Prepared Subgrade
Compact said Crushed Stone
Crushed Stone Base now ready for Footing installation
