ARCHITECT’S SITE OBSERVATION REPORT
DATE OF VISIT: 08/05/2021 PROJECT: Dennis Yarmouth Intermediate
Middle School
286 Station Avenue
South Yarmouth, MA 02664
DATE OF ISSUE: 08/09/2021 PROJECT NO: 71011
WEATHER
CONDITIONS:
CLOUDY, RAINY, 70s ARCHITECT: Perkins Eastman, DPC
20 Ashburton Street
8th floor
Boston, MA
The purpose of the Site Observation Report is to record general observations of the work in progress at the time of the Architect’s visit.
WORK IN PROGRESS:
1) Excavation and subgrade prepping for foundations in A and B.
2) (in AM) Preparation for placing concrete for walls and footings (C and B) (work suspended due to rain).
3) Dense grading at east access road.
4) Excavation and laying pipe for sewer force main.
5) Backfill and compaction from manhole 2 to manhole 3 at electrical duct bank for parking lot lights.
FIELD OBSERVATIONS:
PE visited the site to review ongoing progress with the site work and foundation work.
1. RBO has recently prepared footing lines at areas B and A, including lines at the south area of the gym and was in
the process of preparing line W from A1 to A9 on Thursday (see also attached GPI report).
2. Up until they suspended work in the morning due to heavy rain, Marguerite had been making concrete placement
preparations for footings in the J / J.7 area of the gym, as well as walls on line H.
3. Up until the rain, National Grid had been on site installing the gas main, but they suspended work.
4. The footprint of the building at Area D inside the foundations has been backfilled and leveled with structural fill;
the work was done in accordance with the GAC directive to keep the backfill 12” below TOW; according to
Commodore the underground plumbing work at Area D is scheduled to commence on Monday August 9, with
underground electrical work to follow immediately behind.
5. Dense grade for surfacing the east road (including above UGI 3 and UGI 7) was done on Wednesday 8-4, and at
the time of observation this was ongoing at the very north end of the east access road.
6. The concrete pad for the exterior wall mockup panel has been placed (located just east of the SAE east parking
lot).
7. The following structures have been installed around UGI#1: Manholes #25 and #28, Catch basins #19 and #24.
8. Some trees within the limit of work were cleared on Wednesday for the force main extension to the existing
treatment plant behind the High School.
9. RBO has installed sewer force main pipe from the lift station to limit of work (fence), and RBO is currently in the
process of excavating and laying pipe to extend the force main from there to the treatment plant (see photo). An
existing HP line was damaged in the process but will be repaired by RBO.
10. Vibratory stone column installation has been completed, and as of 8-29 CNC is no longer on site.

FOOTING PREPARATION:
THE SEQUENCE OF PROCEDURES ARE IDENTICAL FOR ALL INSTANCES OF PREPARATION, AND ARE AS FOLLOWS:
Excavate to six inches below proposed Bottom of Footing Elevation
Compact the exposed subgrade, Piers now revealed – with a heavy vibratory plate compactor
Place six inches of 3/4” Crushed Stone upon the Prepared Subgrade
Compact said Crushed Stone
Crushed Stone Base now ready for Footing installation
Activity for Thursday, July 29th, 2021: AREA “B”
Footing Subgrade Compaction / Preparation Detail :
The soil matrix encountered at subgrade elevation largely consists of uniform sands and fine sands with scant traces of
non‐plastic silt. Therefore, the proper in‐situ compaction of this material is largely dependent on the relative moisture
content of the soil. Since the encountered materials are of native, (natural) deposition the water content can vary from
location to location. Each situation must be evaluated to determine that the proper soil compaction techniques are
utilized vis‐à‐vis the moisture content of the soil. In other words, it may be necessary to moisten some areas with
supplied water in order to certify proper compaction in these circumstances.
