The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting. No new information may be accepted into the Commissioners’ packets within the week before a meeting. Failure to follow this Checklist shall result in an Administrative Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting and fee schedule. Contact the Conservation Office if you need assistance. Submitting an Application: 1 original application with original signatures (double sided). A detailed project narrative to include sufficient information to enable the Commission to determine whether the proposed work will alter an area subject to protection (in hardcopy form). BVW delineation sheet(s), coastal bank calculation and other relevant material (in hardcopy form). 1 hardcopy of a Yarmouth Assessor’s locator map highlighting parcel(s) where work is proposed, including map and parcel numbers. Assessor’s maps can be found at www.yarmouth.ma.us/70/Assessor If the applicant is not the property owner, please submit a signed Site Access Authorization Form; AND The owner must be sent a copy of the application via Certified Mail on the same day it is filed with this office. The proposed project is located only within FEMA Floodzone AE, which currently does not have any performance standards, however mechanical flood vents, a 500 gallon drywell, and trench drains will be installed on the property to control stormwater on the Site. The Site is already developed and no adverse impacts to FEMA Floodzone are anticipated as a result of this work. 3. a. Request for Determination of Applicability 185 Seaview Avenue South Yarmouth, MA PROJECT DESCRIPTION 1.0 INTRODUCTION The proposed project at 185 Seaview Avenue in South Yarmouth (the Site) consists of the permitting of a raze and replacement of an existing single-family dwelling with a three-bedroom single-family dwelling that is in compliance with the floodzone standards for this area (the Project). This application is being submitted in accordance with the Massachusetts Wetlands Protection Act (WPA) and the Town of Yarmouth Wetlands Protection Bylaw (the Yarmouth Bylaw). 2.0 SITE DESCRIPTION The property located at 185 Seaview Avenue in South Yarmouth, is bordered by private residences and commercial developments. The lot is currently developed with a single-family dwelling. The wetland resource areas and areas of conservation jurisdiction on or adjacent to the Site, that are protected under the Wetlands Protection Act and the Town of Yarmouth Wetland Protection Bylaw include Land Subject to Coastal Storm Flowage (LSCSF), Zone AE, elevation 11. A wetland delineation conducted by BSC Group revealed no other jurisdictional resource areas on or within 100-feet of the property. 3.0 PROPOSED ACTIVITIES The purpose of this application is to authorize the following proposed activities: Proposed Raze and Replacement of Single-family Dwelling: The proposed project involves the raze and replacement of an existing single-family dwelling with a 3-bedroom, raised single- family dwelling, with a portion of the structure lifted on pilings, and the rest of the structure to be raised 5 feet. Various stormwater features including a trench drain along the eastern face of the dwelling, mechanical flood vents along the western face of the dwelling, and a 500-gallon drywell are proposed to control stormwater on the site. The eastern half of the dwelling will be raised by 5 feet, while the western half of the dwelling will be raised on pilings to bring the structure into compliance with the floodzone standards for this area. The existing septic system will be left in place as is. As the property is already developed and various stormwater control features are proposed as a part of the design, no adverse impacts to LSCSF are anticipated as a result of this work. No vegetation clearing or disturbance of previous undisturbed areas is proposed. Further, any disturbance of soils on Site will be stabilized with grass seed and any other measures necessary immediately following work activities. 4.0 MITIGATION All construction materials will be staged in developed areas. All debris from the Site will be contained in roll-off container stored in upland areas and will be removed from the Site in a timely manner. Any areas where soils are disturbed during construction will be seeded with grass seed and stabilized immediately following work.
