Description of Proposed Work:
Existing double -hung window in the bathroom, on the right of the front entry door, will be removed and
replaced/relocated with a casement window. in order to match and be symmetrical, the existing
double -hung window to the left of the front entry door will also be removed and replaced/relocated. Both
new windows will be 30" from their adjacent walls. New door at right gable not visable from road.

WAIVER OF 45 -DAY DETERMINATION

National and State public health cjnergencics the determination of our Application for a
Certificate of Appropriateness/Demolition/Exemption may not he made within 45 days of the
filing of such application.
The applicant agrees to extend the time frame within which a determination is to be made as
