To the Conservation Commission,
Attached please find Form 8A and a check in the amount of $75 for a Certificate of Compliance for
bulkhead repair at 165 Mayflower Terrace, South Yarmouth.
The project represents replacement of a formerly existing low profile wood bulkhead. The replacement
bulkhead was installed in compliance with the approved plan with the 2 following exceptions.
The wall extends north of the existing stairway approximately 12 ft. This choice for a north extension
was likely driven by observed erosion that would have been included in the original design. The
bulkhead construction occurred quite a few years after the original approval. The low height wall is a
cantilevered wall as constructed, unanchored and appears stable.
Given the stable site conditions and minor, necessary added length, I certify that the project meets the
requirements for a Certificate of Compliance.

subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DER Regional
