All proposed work is now complete, and the dock and plantings meet the intent of the order of conditions with two exceptions: When the project was first implemented for construction, the conservation agent requested that the dock be extended approximately 10 feet landward so that it would span both the salt marsh and the vegetated buffer. The dock was extended, and the stairs shown on the plan were converted to a ramp that extends onto the lawn. The as-built plan shows this extension, with the dock extended to where the stairs were supposed to end, and the ramp extending further onto the lawn. A site inspection was performed by Shorefront Consulting on July 7, 2021. The As-built survey was performed, and when the plan came back in September, the float wasn’t in the correct position (too far landward). The agent was informed, and the decision was made to re-position the float into the correct location. The contractor re- positioned the piles and the float in November. Shorefront Consulting re-inspected the site on December 2, 2021 and found the float to be in compliance. Therefore, the project has been completed in accordance with the Order of Conditions, and all Special Conditions contained in SE83-2220 issued on 4/30/20. Attached please find Yarmouth form WPA-8A Request for Certificate of Compliance, photos of the completed project, a stamped as-built plan dated 12/2/21, and a check for $75 made payable to the Town of Yarmouth. Please include this request in your next public hearing. The original Certificate of Compliance should be mailed to Shorefront Consulting for recording.
