The house work was completed around 1993, as evidenced in the attached Occupancy Permit dated March 5, 1993. The order of conditions was reviewed, and a site inspection was performed by Shorefront Consulting on January 12, 2022. The order of conditions did not require an as-built plan. There was a building permit issued for a deck on the water side of the house, but this was never built. All other dimensions appear to match the 1992 site plan. The attached pictures show conformance to the downspouts being routed to drywells as indicated in condition #18. However, the driveway is now paved, which is not in compliance with condition #17. A review of Google Earth aerials does not indicate when the driveway was paved, but the photos do show that all but one driveway on the street is either paved, or with pavers back to at least 2001, and the 1995 aerial not being quite clear enough to make a determination. Therefore, the driveway being paved is not out-of-context for the street, and not likely for the neighborhood. The driveway slopes towards the street, so any runoff is directed into any town drainage structures and not any resource area. This appears to be the only deviation from the order. Therefore, with one exception noted, the project has been completed in accordance with the Order of Conditions, and all Special Conditions contained in SE83-1142 issued on 12/26/1991. Attached please find Yarmouth form WPA-8A Request for Certificate of Compliance, photos of the completed project, and a check for $150 made payable to the Town of Yarmouth which covers the fee for both COC requests. Please include this request in your next public hearing. The original Certificate of Compliance should be mailed to Shorefront Consulting for recording.
