On behalf of my client, Patricia Dresnek, please accept this request for a Certificate of Compliance for the above
referenced project. The dock work was completed, but Google Earth does not show it clearly until at least
December of 2001 (landing only; winter), and not clearly until 2010. The order of conditions was reviewed, and
a site inspection was performed by Shorefront Consulting on January 12, 2022. The order of conditions did not
require an as-built plan.
The pier, ramp, floats and bulkhead have been measured and found to be in compliance with one exception. The
ramp is 2.5 feet wide, but 14 feet long instead of 12. The distance for the floats from the east property line shown
on the 1994 permit plan appears to be 27 feet. The float was measured in Google Earth and conforms to this
dimension. Therefore, the floats are where they are supposed to be; the ramp just overhangs them more than the
permit plan shows. This does not affect the permitted footprint because the ramp lands on the float. The ramp
was likely extended to reduce the angle at low tide. This is a common condition, and has no effect on the
compliance of the project.
There were no holes in the bulkhead, and the required 10’ of buffer strip is in place. Therefore, with one exception
noted, the project has been completed in accordance with the Order of Conditions, and all Special Conditions
contained in SE83-1289 issued on 7/14/1994. Attached please find Yarmouth form WPA-8A Request for
Certificate of Compliance, photos of the completed project, and a check for $150 made payable to the Town of
Yarmouth which covers the fee for both COC requests.
Please include this request in your next public hearing. The original Certificate of Compliance should be mailed
to Shorefront Consulting for recording. If there are any questions or concerns regarding this request, please

Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
